What do I bring to an assessment? 1. You need to bring all required paperwork. This paperwork can include: BAC (Blood Alcohol Concentration) Judgment Papers Citation Any court-related paperwork (DWI, divorce, custody, etc.) 2. You need to bring a list of any medication you may be taking. 3. You need to bring your insurance card or insurance information. 4. Also, remember to bring your assessment fees and money orders. General $100 DWI $100 + $10 for Driving Record DWI Screening greater than 6 months $100 Out of State DWI $100 + $75 processing fee ____________________ |
